We are seeking a dedicated and compassionate Social Worker to join our Social Work and Community Support Service (SWCW) at Jewish Care. In this role, you will collaborate with individuals, their carers, and various health and social care professionals to provide holistic support that promotes independence, choice, and control. Your work will focus on addressing difficulties related to ageing, caring responsibilities, illness, isolation, disability, mental health issues, or changes in circumstances within the Jewish Community.

As a Social Worker, you will conduct comprehensive assessments, identify key areas of support, and work in a multi-disciplinary approach. You will actively engage in community projects, represent Jewish Care in various settings, and contribute to the development of services and resources. Supervision and support for team members and volunteers will also be part of your responsibilities.

This post is full time for 36.25 hours per week, and will require access to a vehicle for work.

What you will be doing:

  • Working collaboratively with individuals, carers, and relevant resources to provide personalised care, reducing risk and improving quality of life.
  • Actively participating in collaborative and multi-disciplinary approaches, building networks within and external to Jewish Care.
  • Supporting the development of community and group interventions to enhance social and community life.
  • Contributing to forums focusing on Dementia, Disability, Family Carers, Mental Health, Safeguarding, Mental Capacity, End of Life Care, and Holocaust Survivors.
  • Working with the Team Manager and Senior Social Workers to develop and run an effective service.
  • Developing relationships within Jewish Care, Statutory, and Voluntary organizations.
  • Presenting the work of the Social Work and Community Support Service in various forums.
  • Providing information and advice following current government health and social care guidance.
  • Supervising team members and volunteers, managing your own case load.

What you will need:

  • Qualification in Social Work (E)
  • Registered Social Worker with Social Work England (E)
  • Experience working with diverse populations facing Mental Health, Mental Capacity, Disability, Dementia, Isolation, End of Life, and other co-morbidities (E)
  • Experience in community and/or group work (E)
  • Experience working with adults in a social care context (E)
  • Knowledge and experience of Safeguarding, Care Act (2014), and Mental Capacity Act (2005) (E)
  • Knowledge of social work interventions with individuals, families, groups, and communities (E)
  • Knowledge of working with people experiencing Mental Health, Disability, Dementia, Isolation, End of Life, and other co-morbidities (E)
  • Ability to apply critical reflection and up-to-date knowledge to all aspects of the role (E)
  • Ability to use electronic databases for maintaining up-to-date client records (E)
  • Excellent verbal, listening, writing, time management, and IT skills (E)
  • Ability to work independently and flexibly within a multi-disciplinary team, including hybrid working (E)
  • Ability to apply ethical principles to decision-making and support others in doing so (E)
